_By default_.  Maybe, yes.  Some mailing lists are set up like that;
many are not.  But either way a subscriber should have the option to
reply to the author only, and Reply-To use by the ML software works
against that.

Depending on the mailing list it is the *usual* action, or 50-50, or
10%, or sometimes even 100%; but almost never 0%.
